In order to provide prompt service, you can download the attached Referral form. It is important that recent blood work be provided prior to your patient undergoing anesthesia.

Instructions for Referring Veterinarian:

      1. Click here to download the referral AND CONSENT FORM (which is in PDF format) for the patient's MRI procedure.

      2. Fax the form to us.

      3. Send the current blood work performed in the past three weeks.
